Make sure you're fully releasing the trigger after the first shot, same thing was happening to a mate with a new gun and this turned out to be the problem,the gun wasn't resetting due to (slight) pressure on the trigger.

Yep moving the selector or putting the safety on and off again will usually reset the second barrel on most inertia block shotties.
The second barrel not firing for some people with light loads is quite a common problem. The inertia block spring can be lightened if you are only likely to use light clay loads but I would check your mount and technique first. It is very unlikely that one barrel is not cocking as it is virtually impossible not to open a Beretta fully when putting in the cartridges.

I had the same issue with my Silver Pigeon 1 last year, it went back and they adjusted the recoil mechanism in the gun. Got it back and it has done the same thing once since, as has been suggested it may be that I am not fully releasing the trigger bewteen barrels. A bit of a pian in the butt, but I am not bothering to send it back, just operator error. My loads are heavy enough so that is not a issue either...

Well, there wasn't much to shoot at yesterday and I don't know if it deffo was me not releasing the trigger causing the problem. However, there were no problems so i think i'll just disappear back under my stone 'til the next time. ;)

Obviously a transient problem which had been caused by a small particle of grit which then dislodged itself when you stood the shotgun in the cabinet between shoots :thup: :thup: :thup: :grin:
